What exactly are you going for with this guy? Because, I gotta be honest, "Elite" seems a little funny when paired with a +4 Attack...
I'm not sure how I feel about Triple Attack on a non-unique. WotC has only done that a half dozen times, and with the exception of the Rakghoul, all are significantly more expensive than this piece. I spoose Heavy Weapon is a decent balance, but the double whammy of Mighty Swing and CQF sort of throws that balancing effect away.
Pair him with a CzerkSci and some sort of swapping ability, and he can dish out 6 attacks of 20 damage at +8, and that's without any other support. It's not tough to drive the damage to 30 or even 40 per attack, which is pretty crazy.

I was going for a Fringe character with a Gattling Gun (Triple Attack).
Perhaps I should remove Mighty Swing and/or CQF?

If you're going for a "Gatling gun," I'd say the melee-based special abilities don't need to be there.

With all these "Kota's Militia" pieces flying out, I feel like I should toss out my pieces. I made a full platoon's worth, with a Platoon leader, sergeant, corporal, militia trroper, repeater gunmen, elite vanguardsmen, tech and comm specialists, etc. ... won't do it unless Shar wants someting to compare/contrast his with ...
I also might as well toss my 2 pennies worth on Fringe; I also made them Fringe, but with Affinity - this character may only be in a squad with a character named Kota ( guess you could call him Rebel with Affinity for Republic or vice-versa.
For the Record, a typical Kota's Militia piece
Militia Trooper Count's as Kota's Militia
HP: 40 Def: 14 Atk: +8 Dmg: 10
Affinity - This character may only be in a squad with a character named Kota Opportunist Stealth
Bass for all KM was 40HP, Df14; Attack and Dmg depended on what the piece was ...... Opportunist because they took the initiative an attacked when They wanted, staelth because they're using gurilla tactics .....

If the intent is that these guys are supposed to work with Kota (from, y'know, the name) and both versions of Kota are Rebel, why would they be Fringe? I can see an Affinity, either for Kota or the Republic, but Fringe doesn't seem to make sense to me. And, to be honest, I don't know that I'd even need to see Affinity. Looking at Sharrakor's and WotC's versions of Kota, I'd assume that the WotC version is "first," when he's forming his militia (who are recruited Fringers, hence the commander effect). Sharrakor's version, firmly Rebel, is once he's soldified his militia into a true and loyal fighting force against the Empire (hence, they're now Rebels and CALLED Kota's Militia). I mean, you wouldn't have the Emperor's Shadow Guard be Fringe, would ya?

This is fascinating. The +4 Attack makes the Triple acceptable. This is a really, really interesting piece: I really like the dynamic of the Heavy Weapon on there.

I don't think it's all that viable, I mean with HW you have to win init or this guy dies. Period. And, if you happen to get lucky enough to win init, you get to make 3 10 damage attacks, at +4 attack. If you have a Czerka, it's 6 shots, which would be okay, if it didn't have a +4 attack. This remind you of a certain KotOR gunslinger that sees absolutely no use for similar reasons?
